GP practices across West London CCG have formed local ‘Primary Care Networks, to improve the services we provide to patients.
This practice is part of Brompton Health PCN; a group of practices, located across the southern half of Kensington and Chelsea. We have come together because we share a common aim to improve the service we provide to our patients, and a wish to work more closely together to achieve this aim.
Our doctors regularly offer referral into the service. Alternatively please advise your doctor should you wish to be referred in. Patient consent is required to make the referral and be able to share your medical records with our My Care, My Way staff.
If you are already under our My Care, My Way team and need to make contact with them regarding any matter please contact
